Wallkill Golf Club - 40 Sands Rd, Middletown, NY 10941
Shot-Gun start 9:00 - Check-in and breakfast 7:30-8:30
2 Person Scramble -
Foursome shoot off the same tees (must be of same gender)
$140 per player (Checks made out payable to Bleu's K9 Rescue or Venmo Christina Palaez@tpelaez or Cash)
100% of the funds go to Bleu's K9 Rescue!

NOTE:
- Please bring cash to the tournament. If you want to buy Mulligans - 2 per player - 1 to be used on front nine and one to be used on back nine for only shots off the tee. $5.00 per Mulligan.
- 3 contest holes for each gender: $10.00/per contest hole or $20 for all 3 contests. the contests are: par 3closest to the pin, longest drive, and losest to the line. $100 cash prize for each winner. (You may pay for this whenyou check in at Wallkill Golf Club as well)
- There will also be a Par 3 "Beat the Pro" contest. It will be $10.00 to enter that contest. If you beat the pro, you get your $10's back + $10 more.
- Finally, there will be a raffle of prizes before lunch. There will also be a tiebreaker par 5 hole - Partners use the same lofted club on all shots on this par 5. If there is a tie, for overall winners of this golf tournament, lowest score on this hole wins the tournament. If there is still a tie, the highest lofted club wins (example: 9 irons beat 7 irons)

NOTE: All prices are wholesale and include the cost of the material, labor and firing.
Each piece takes 4 to 6 weeks to complete. Please allow time for personalized pieces to be made. A ton of love and energy goes into each piece personally.
BLEU'S K9 RESCUE WILL RECEIVE 10% OF EACH ITEM SOLD.
To get started:
1. Choose what you would like the portrait painted on (mug, plate, etc.)
2. Pay a 50% deposit when you send in your pet's photo by VENMOing: @shannon_sforza
3. Then email her any details along with the photo of your pet to: sforzashannon@gmail.com
4. Use the same email address to firm up a date and location to pick up your artwork
5. Pay the difference at the time of pick-up
